The role of sex and neighborhood disadvantage in cognition and blood-based AD biomarkers in a racially and ethnically diverse community cohort

Objective:

Women show more frequent diagnosis and poorer clinical course in Alzheimer’s Disease (AD), compared to men. These sex-based differences stem from multiple interactive causes and are critical to consider when examining AD outcomes such as memory and brain volume, and when using predictors such as inflammation and amyloid/tau/neurodegeneration biomarkers. Despite the growing body of literature on AD sex differences, most studies have been conducted in cohorts that include predominantly non-Hispanic White (NHW) participants, and without consideration of the community context. This analysis examines the impact of sex and neighborhood disadvantage (ND) on cognitive, AD biomarker, and inflammation-related outcomes in a large community cohort of NHW and Hispanic individuals.

Participants and Methods:

Analyses included baseline data from the Health and Aging Brain Study—Health Disparities (HABS-HD), from 417 NHW individuals (225 women) and 657 Hispanic individuals (448 women). Neighborhood disadvantage was assessed via Area Deprivation Index (ADI), collapsing state deciles into quintiles. We compared those in the least (ADI 1-2) and most (ADI 4-5) disadvantage areas. Univariate generalized linear models were used to examine effects of sex, ADI, and their interaction on verbal learning and recall, plasma tau 181 (ptau181), and inflammation-related outcomes (IL-5, IL-6, IL-10, TNFα), separately for NHW and Hispanic participants. For all analyses, age and education were included as covariates; body mass index was additionally included for ptau181 and inflammation-related analyses.

Results:

Consistent with prior studies, women showed better verbal learning and recall than men in both the NHW and Hispanic cohorts. In addition, Hispanic women—but not Hispanic men—fared worse on verbal learning if they also lived in a disadvantaged area. No interaction of ND and sex, and no main effect of ND, was observed for NHW individuals. Hispanic women showed lower levels of ptau181 compared to Hispanic men; this finding was not observed in NHW women. A trend was observed toward higher ptau181 in NHW individuals with greater ND; no effect was found in Hispanic individuals. Both NHW and Hispanic women showed lower IL-10 and IL-5 levels compared to men regardless of ND. Trends emerged in Hispanic, but not NHW individuals, showing interaction between sex and ND for IL-6 and TNFα. In both cases, women showed higher levels of the inflammatory marker compared to men, but only at low levels of neighborhood disadvantage.

Conclusions:

NHW and Hispanic women show expected advantages in memory compared to men, though Hispanic women’s memory may be more vulnerable to neighborhood disadvantage. Sex related to ptau181 only in Hispanic individuals. Results for inflammation-related outcomes require further investigation but underscore the importance of including sex as a predictor, particularly for participants with low neighborhood disadvantage.
