Objective:

Utilization of patient cases within an outpatient neuropsychological context to address unique clinical considerations and complications of FND with co-occurring diagnoses and in order to understand continued impact of diagnosing patients with FND.

Results:

Reviews of information gleaned through Case 1 revealed factors that complicate case conceptualization including origin and sequalae of electrical injury, reliability of differential diagnosis of neurogenic stuttering based on known classification models, complexity of neuropathological correlates of acquired stuttering in conjunction with lack of structural brain changes on imaging. Case conceptualization complications for Case 2 included cross-assessment inconsistencies, known variance in performance validity test failure rates in FND, dichotomous decision making within a worker’s compensation context, and psychosocial factors that may contribute to level of independent functioning and origin of symptoms.

Conclusions:

Larger implications exist involving historical underpinnings of FND and further emphasize the importance of clinical decision making within the context of ethical standards and best practice as well as the role of neuropsychologists as ethical decision making entities and as exemplars in the field of medicine and psychology as consultation-based demands increase.
