Tittle: Clinical Integration of An Evidence Based Self-Management and Cognitive Training Program for Epilepsy

Objective:

Epilepsy self-management (ESM) is the summative set of behaviors that people with epilepsy use to cope with their disease and optimize their overall health. Self-management challenges facing adults with epilepsy include limited understanding of the condition and treatment, and associated psychosocial issues. Despite mounting evidence of the benefits of ESM programs, bringing ESM programs to clinical centers has been challenging.  Dartmouth Health’s HOBSCOTCH Institute for Cognitive Health and Well-Being has made measurable progress in addressing neurocognitive difficulties and improving the quality of life in people with epilepsy with a centralized support facilitating the dissemination and implementation of the ESM intervention, HOBSCOTCH (HOme Based Self-Management and COgnitive Training CHanges Lives).

Participants and Methods:

HOBSCOTCH is an evidence-based, telehealth administered program delivered one-on-one by trained clinicians.  It is designed to help patients learn to manage and cope with cognitive problems leading to improvements in quality of life. The 8-week intervention fosters participants’ understanding of foundational concepts about cognition in general and in relation to their epilepsy, and assists them in building metacognitive awareness to identify situations and conditions under which their cognitive challenges occur.

Results:

HOBSCOTCH helps patients build skills through Problem-Solving Therapy, implementation of compensatory cognitive strategies and mindfulness techniques.  The problem-solving framework empowers patients to feel more in control of their challenges as they drive the innovative solutions applied to their self-identified problems and goals throughout the program.

Conclusions:

Opportunities for provider training and integration of HOBSCOTCH at epilepsy centers and in private practice settings is supported by a multidisciplinary team at the HOBSCOTCH Institute.
